Hypnotherapy/Counselling Contract 2005

1. I will Adhere to the national Register of Hypnotherapy and Psychotherapy (NRHP) and the United Kingdom council for psychotherapy (UKCP) code of conduct. Please see web sights.

2.Confidentiality will be assured.

3. I will have regular supervision with an accredited UKCP supervisor to maintain the quality of my work.

4.INITIAL TELEPHONE CONSULTATION IS FREE. Should you wish to proceed Hypnotherapy sessions are charged at £45 (except for smoking, charged at £149 for 2 sessions and a follow up session if required. ) and Counselling/Psychotherapy are charged at £35.

5.At the first session I WILL ASK FOR ONE SESSIONS PAYMENT IN ADVANCE.

6.24 hrs cancellation notification must be given in order to receive a FULL REFUND otherwise the fee will be forfeit. I will also give 24 hrs cancellation notification. If i do not then you will receive a free session.

7. BY PRESENTING FOR THERAPY IT IS ASSUMED THAT YOU ARE WILLING TO EXPLORE OPENLY AND HONESTLY YOUR ISSUE(S) AND THAT YOU ARE PREPARED TO WORK TOWARDS CHANGE.

8.THIS CONTRACT AIM'S TO SET THE 'GROUND RULES' AND 'BOUNDARIES' ON WHICH THERAPY IS BASED. IT IS FLEXIBLE AND OPEN FOR RENEGOTIATION AT ANYTIME DURING THE THERAPY PROCESS.
